Volere template and process now has a dedicated website at

includes useful resources and downloadable versions of the

Here you will find a set of questions (and answers) about
questionnaires. The main thrust of these is for usability
evaluations, but some of the issues are suitable and generally
applicable to questionnaires and their design for the requirements
activity.

Alistair Cockburn has been working with use cases, and with
people-oriented concerns in software development for a long
time. His website includes an article about how to structure
use cases with goals, and some subsequent discussion about
it. You'll also find some interesting reflections on human
factors in software projects.
